Create new game page revamped

The Create New Game page has been optimized a bit.

If you are a non-premium player, all options in the right table will be 'greyed out' and a message is displayed that you need to be a Premium member to be able to change them.

Another pedantic quibble.....

On the mouse-over text for regular games, there are references to 'shoutbox'. Since we have replaced shoutbox with public press, could these references be changed too?
